This is a records release form to be used prior to registering with the NCAA Clearing House. No transcripts may be released without prior consent. Athletic Records Release Form End of junior year/beginning of senior year register for the ClearingHouse To register for the NCAA ClearingHouse click on the following link: https://web1.ncaa.org/eligibilitycenter/common/ Click on the: Prospective Student Athletes Follow the prompts from there to register. Once registered, download the Transcript Release Authorization, student and parent sign and bring to Ms. Ward in Guidance with $6.00.This will pay for the transcript that is sent to the Clearing House initially and also the final transcript. They will be given a student release form from Mrs. Ward that needs to be brought home and signed by parents. Return this to Mrs. Ward. That release form allows Mrs. Ward to give transcripts to coaches that inquire about the student athlete.
